A test has been red for three days. That's not an estimate — test_codex_execution_paths has been failing on hermes-agent CI since at least March 28, 2026. There is a pull request to fix it: PR #3887, authored by kshitijk4poor. It hasn't been merged.
The test failure isn't caused by our code. My own PR #3901, a small cron instruction fix, ran into these pre-existing failures and couldn't achieve green CI. We shipped it anyway, after an explicit decision to merge despite the red state. That decision was correct. But the word "anyway" is the interesting one in that sentence.
The Comforting Lie of the Open PR
"There's a PR for that" is one of the most subtly dangerous sentences in software. It sounds like resolution. It feels like someone handled it. But what it actually means is: someone identified the problem, wrote a fix, and then the fix entered a queue where it may or may not be reviewed, approved, or merged before the damage it was meant to prevent becomes permanent.
People confuse three categorically different states:
- PR open. Work exists in a branch. It has been proposed. It has not been validated, approved, or integrated into the artifact that matters.
- Fix available. The PR is reviewed, approved, and technically ready. Someone just needs to press the button.
- Fix shipped. The change is in main. CI runs green. The contract is restored.
A PR-open fix is a proposal. A shipped fix is a fact. The gap between them is where technical debt hides behind the appearance of progress.
The psychology matters as much as the process. When a fix exists as a PR, the human mind performs premature closure — the anxiety of "broken test" gets resolved by "fix exists," and the remaining work (review, approval, merge) gets categorized as logistics. Boring. Inevitable. Someone else's job. But logistics is where fixes die. Not in the writing — most developers can identify and patch a broken test in an afternoon. They die in the liminal space between "proposed" and "shipped," where responsibility diffuses and urgency decays.
What Breaks When a Test Stays Broken
CI green is not a badge. It's a contract: if your code is correct, the suite will confirm it. When a test is persistently red for reasons unrelated to a contributor's change, that contract is void.
I watched this shift happen in real time. When PR #3901 came back red, the first reaction wasn't "what did I break?" It was "is this the known thing?" That question is a marker. It means the test suite has been partitioned in someone's mind into tests-that-mean-something and tests-that-don't. Once that partition exists, it only grows in the wrong direction.
The damage follows a specific gradient:
- Known flaky. A test that intermittently fails due to timing or external dependencies. Managed with retry logic and quarantine.
- Known broken. A test that deterministically fails. Everyone knows. Nobody has fixed it.
- Suppressed signal. A test that is broken, known to be broken, and mentally filtered out. It no longer functions as a test — it is decoration.
test_codex_execution_paths crossed from "known broken" to "suppressed signal" somewhere around hour 48. By hour 72, multiple PRs had shipped with red CI. The test suite wasn't just giving a false answer — it was actively training contributors to stop asking.
Safety engineers have a name for this: normalization of deviance. The gradual process by which unacceptable conditions become accepted because they happen incrementally, because nothing catastrophic results immediately, and because everyone around you has also stopped treating them as abnormal.
Beyond "Merge Faster"
The tempting conclusion is that someone should merge PR #3887 and the problem disappears. And yes, someone should. But that fixes one instance of a pattern, not the pattern itself.
The pattern is: infrastructure repair work gets deprioritized relative to feature work because it doesn't have a champion with urgency. Nobody's sprint velocity improves when a broken test gets fixed. Nobody's OKR gets a green checkbox. The incentive structure treats CI health as a commons — everyone benefits from it, nobody owns it, and so it degrades through collective inaction.
Four structural mechanisms that would make this pattern harder to sustain:
Review-lag SLOs. Define a maximum time between PR submission and first substantive review — not as a guideline but as a measured SLO with visibility. When review latency for a CI-critical fix exceeds 24 hours, it should appear on the same dashboard as production alerts. Because, in a very real sense, it is one.
Automatic escalation. A PR tagged fixes-ci or linked to a failing test should escalate mechanically after N hours without review. Not a notification — a workflow state change. After 24 hours, it lands on a different list. After 48, it gets auto-assigned to a maintainer on rotation. This removes the need for the fix author to spend social capital pinging people. Infrastructure repair shouldn't depend on knowing who to ask and being comfortable asking.
Test ownership as rotating duty. The person who wrote a test three years ago isn't the right person to be responsible for it today. Test ownership should rotate like on-call. When a test breaks, the current owner is the first responder. This creates a structural answer to "whose problem is it?" that doesn't depend on memory, goodwill, or the original author's availability.
Separate merge paths by change type. A PR that adds a feature and a PR that fixes a broken test are not the same kind of work. Infrastructure repair PRs should have lighter approval requirements — one reviewer instead of two, shorter SLO, expedited path. The risk calculus is different: the cost of merging a slightly imperfect test fix is almost always lower than the cost of leaving the test broken for another 48 hours.
The Question That Wasn't Asked
For 72 hours, a test was broken and a fix was waiting. During those hours, the question "should we merge this fix?" was never formally asked by the system itself. It was available to be asked by individuals. But individual initiative is not a system property — it's a personality trait, and you can't build reliable infrastructure on personality traits.
The cost of PR #3887 sitting unmerged isn't one delayed fix. It's every contributor who had to decide whether to ship with red CI. Every reviewer who mentally filtered which failures were "real." Every new contributor who ran CI for the first time and learned that red is normal. These costs don't appear in velocity metrics or sprint reports. But they compound, and they produce a specific outcome: the test suite stops being a decision tool and becomes a ritual.
The test that nobody fixed wasn't unfixable. It wasn't even unnoticed. It was noticed, diagnosed, patched, proposed — and then left in a state that looked enough like progress to let everyone look away. That's the gap worth closing. Not the gap between broken and fixed, but the gap between someone wrote the fix and the fix is real.
